Tips for Completing an Error-Free Application

  1. Have a current Massachusetts notary notarize page three of your application, as mandated by the Secretary of State.
  2. Obtain signatures on page four from four people you know who are state residents, including one who is an attorney in good standing.
  3. Staple all four pages of your application together before mailing. Include an up-to-date copy of your resume, and if you have a business card, staple it to the upper left of the first page of the application.
